Lo3 - Pre production and planning forms.

Once a production schedule is created other forms are needed depending on what you're making, such as: 

- release and content forms.

- location permission forms.

- Call sheet. 

Release and consent forms are legal contracts shown by the signature at the 
bottom of the page, they're used to get permission from models that confirms they agree to being photographed / be a part of the project and have the product distributed.     

Based on this example the main three main sections included within a release and consent form is: confirming who the model is, an explanation of the the agreement and a signature from the model.
Legal issues that could be faced without this form is that the company / photographer could be sued due to no proof of confirmation by the model if they suddenly decided they didn't want to participate on the project anymore. Ethical issues that may be faced without the existence of this form would be that the models may not fully understand the extent in what they have to do due to no explanation / agreement being shown therefore they may feel uncomftable when it comes to the actual photoshoot, they could also sue to company for deformation of character due to this.

Location permission forms are legal contracts, they're used to confirm permission to be / film in a location form the owner of the property. 

The location permission forms have similarities to the release and consent forms due to them both being legal contracts and the fact that you're gaining someones permission for something. 

The person who would sign this form would be the person who owns, manages or is responsible for the property. 

Call sheets are used to inform people working on the project all information in which is relevant to a particular activity within the project such as a photoshoot, they must be given to cast & crew before the event takes place. 

call sheets can be used for productions such as films, magazines, games. Everybody involved within the project has to have a call sheet due to it being relevant to everyone as it includes elements from all different sectors such as makeup, costumes, actors, camera crew etc..Call sheets would be needed to be given to these people before the production starts so that everyone is prepared and know what resources need to be gathered.
